Please make sure that you fill in all the correct information on the form to get a Campus Pass. If the issue persists, please contact the Case Manager or IT department. 

Please fill out the Vaccine report form, which can generally be found under Campus Guide in the Campus App.

If you have already filled out a form that provided you with a red pass, please reach out to the Case Manager of your School to get a green pass.
