Create a Form

To create a Form, you will need 'Forms' permissions on Campus Cloud and have access to a Host.

Navigate to Campus Cloud. Go to Manage > Forms (under CAMPUS CONTENT).

Note: Contact Trace forms are still housed under Manage > Contact Trace > Forms. You cannot create Contact Trace Forms from this Forms area.

Create Your Form

  1. Click 'Create Form.'

  2. Give your form a unique name.

  3. If you want to start from a pre-built template, select a category to see the available templates. Learn more about form templates.

  4. Select a host. This determines who can see/manage the form.

  5. Click 'Next.'

Build Your Form

Welcome Block

Each form is required to begin with a Welcome block. Introduce the topic of the form. You can add a header image or video to this (and every) block in the form.

Form Blocks

Add questions and feedback to your form.

  • Text Explanation: add conditional feedback that relates to a previous question

  • Text: accepts a short text-only answer up to 255 characters

  • Number: accepts only integers with no punctuation (up to 16 digits)

  • Decimal: accepts integers with a decimal point (up to 15 digits)

  • Single Section: users can select only one option (radio buttons)

  • Multiple Section: users can select one or more options (checkboxes)

  • Yes/No: single selection that is pre-set with Yes and No options, which you can edit

  • Date: accepts a single date

  • Image: users can upload a single jpg or png

Result Blocks

Each form must end with at least one Result block. You can have multiple Result blocks, and use conditional logic to send users to the appropriate result. Result blocks are also how you target groups of users when triggering notifications based on form completion in Workflows.

Conditional Logic

Every form block has the option to add “logic.” This is how you set a different “next” block depending on the value the user selects or inputs. You can use this to skip students to “text explanation” blocks that relate to the answer they selected.

This is also how you direct users to the final Result block that matches their input.

Form Templates

Ready Education provides a solution called Interventions+ that includes a collection of 25 form templates covering topics within student life, health and wellness, financial, and academics. Templates are a great way to create forms without having to start from scratch.

These pre-built forms feature engaging imagery, questions with conditional feedback, and a range of different “results” pages that promote resources related to the students’ input. You can use these forms to check the pulse of your students throughout the academic year.

Once you have access to the collection, you can select the category and template when creating your form. You can then edit the pre-populated content and tailor it to your students.

 

Forms Permissions

Under Team Settings, If you have access to Manage > Forms, you will be able to create and see form responses from users. 

If you have access to a set of Hosts (services, organizations, etc), you will be able to view the forms hosted by them.
